NEW HAUORA TARANAKI PHO ROLE - COMMUNITY CARE COORDINATOR

The PHO is introducing a Community Care Coordinator role that will be available as a “single point of contact” for Practice/provider staff to access patient support services (other health providers and community agencies) with the objective of releasing valuable clinical and administrative time. The role will have a particular emphasis on community based service provision.
Process:
The aim of the Community Care Coordinator will be to provide prompt and efficient referrals to the relevant services and/or advice back to the provider teams following a simple phone call to the PHO.
The Practice/provider staff can feedback directly to their patient or ask the Community Care Co-ordinator to contact the patient to facilitate ongoing support.
We have developed this role as we recognize that the constant changing landscape of referral agencies and support services does lead to large amounts of time being spent on finding appropriate support services. We hope a quick phone call to the Coordinator, who will have a finger on the hub of primary and secondary services available will help said PHO Manager Pauline Cruickshank.
